No floormats - which apparently is pretty normal for rental cars.
I didn't really effect me, but with New England weather, I was worried about the snow and salt ruining the carpet.
They suggested that I take a taxi to another location with a car two hours away (and two hours farther from my final destination) - as if I had all the time in the world to drive around the country making up for their mistake.
(Another couple, that had reserved a car to drive to Boston, was told they could pick up a car in Boston.) They refused to guarantee me a car locally the next morning or offer any financial compensation.
I showed up at the time of my reservation only to be told there was no car available within a 100-mile radius.
When I called customer service (the guy at the desk was apologetic, but he could do nothing), the conversation was Kafkaesque.

As if they were being extremely generous, they did offer to cancel the reservation!
Luckily, another local company had a car - but I'm never using Avis again.
